Forms and Documents

We publish the following forms on this web site for our members' use. Each form includes instructions and any applicable policies regarding its use. Please refer to the form for the procedures to follow in completing and submitting the form.

Any form that is to be submitted to the church office can be emailed to office@stmark-lutheran.org. We recommend that you follow-up personally with the church Office Manager within 2 days of submitting your form via email.  

Important: When you select a MS Word form, you will be prompted to "Open" or "Save" the form. You must select" Save" to save the form on your PC. You can them complete and print the form as required.
